What are the Challenges in Pathology Report Transcription? Transcribing pathology reports is a task that demands precision and expertise. We confront various challenges, including: 1. Complex Medical Terminology: Pathology reports are a puzzle of medical terms and scientific jargon. Accurate transcription is essential for patient care. 2. Handwriting Mysteries: Pathologists' handwriting can be as cryptic as a treasure map. Deciphering it accurately is crucial to avoid errors. 3. Diverse Report Formats: Pathology reports come in all shapes and sizes, often lacking standardization. Adapting to these variations while maintaining data integrity is a must. 4. Diagnostic Complexity: Pathology reports span a spectrum of medical conditions, from simple to highly intricate diagnoses. Transcriptionists must be versed in this diversity. 5. EHR Integration: Seamlessly integrating reports into electronic health records is vital for continuous patient care. We navigate various EHR systems and their unique requirements. 6. Data Security and Privacy: Patient confidentiality is paramount. Strict security protocols and adherence to regulations like HIPAA are crucial. Continuous Learning: Overcoming these challenges requires ongoing education, domain knowledge, and attention to detail. “In medicine, we can not tolerate hallucinations,” said Dr. Gregory Ator, the chief medical informatics officer at the University of Kansas Medical Center in the U.S., which is not using generative A.I. in diagnosis yet because the software can create fabrications. However, according to a feature story by the venerable tech reporter Steve Lohr published by The New York Times this week, the advanced A.I. technology is saving doctors a lot of time with paperwork. For example, Dr. Matthew Hitchcock, a family physician in Chattanooga, Tenn., is using an A.I. helper to summarize treatment plans and billing each day. What used to take him up to two hours at the end of the day, now takes him only about 20 minutes or so. Typing up these medical notes every day after his four children went to bed is now “a thing of the past,” said Dr. Hitchcock. “It’s quite awesome.” While A.I may someday help us uncover medical miracles – for now, it’s helping healthcare practitioners ease the burden of documentation that takes them hours a day and contributes to burnout. What are these and why should YOU know these as a medical interpreter? These are the basics! If you know these terms you'll be able to know what comes next in that medical consult you are interpreting for ;) ☑ Diagnosis (Dx): the identification of the nature of an illness or other problem by examination of the symptoms. ☑ Differential Diagnosis (DDx): A differential diagnosis process is used to help make a final diagnosis when you have symptoms that don't have one clear cause. Perhaps you have several symptoms that can be a sign of several illnesses! ☑ Prognosis: is a prediction about the course of a disease. To be able to provide a Dx or DDx, doctors need to check both the objective and subjective terms: ☑ Objective medical evidence means signs, laboratory findings, or both, from a medical source. ☑ Subjective terms are the ones provided by the patient: "I have pain in the lower back and I feel dizzy when I get up". PAY ATTENTION TO THE SUBJECTIVE TERMS, some LEPs may use strange terms to refer to these like saying "dolor de estómago" instead of "tummy ache" (upper abdomen instead of lower abdomen). AND ALWAYS KNOW WHEN TO ADVOCATE FOR THE PATIENT YOU ARE INTERPRETING FOR
